∗ ∗ Patients will be discharged after two no-call/no-shows. Appointment reschedules or cancellations are to be made at least 24 hours in advance, otherwise there will be a $25 charge.

∗ ∗ All patients including those on controlled substances that have not been seen in office by the provider for six months may be discharged.

∗ ∗ Medical forms to be completed by provider including Jobs Plus forms, forms from Social Security, and forms from the Department of Social Services are not able to be processed immediately and may require an appointment for completion depending on the information requested.

∗ ∗ Effectively immediately, no prescriptions will be refilled for clients with outstanding balances. I am under obligation to perform state mandated steps before ordering controlled medications. This is a time-consuming process for the provider and requires documentation which is billed to the insurance company and the patient is responsible for any balance due. By submitting the billing code this also informs your insurance plan that I have completed the mandated obligations as a provider prior to prescribing controlled medication. Each time there is a controlled medication(s) ordered you will incur an appropriate fee.
